What ages does Dr. Bajema treat?

Sage Health treats patients 18 years and older.

Do you accept insurance?

No, Sage Health does not bill or accept payments from insurance companies.

Do I still need insurance?

Sage Health is not an insurance program. We recommend that you carry healthcare insurance to cover other expenses such as emergency services, hospitalizations, specialists, and laboratory, imaging, and pharmacy services.

What if I decide to cancel my membership?

You can cancel at any time, however there is a minimum 3-month commitment to the Concierge Practice.

Can I use my Flex Spending (FSA) or Health Savings Account (HSA) card for membership payment?

Concierge practice subscription fees are not an eligible expense for FSA or HSA reimbursement. However some of the program services provided as part of your membership, such as physician fees for medical exams, may qualify as reimbursable expenses under the IRS rules. We can provide you with detailed invoices for your doctor visits. You’ll need to verify this with your plan administrator and your tax professional.

What is your policy on controlled substances?

Sage Health does not utilize opioids for the management of chronic pain conditions. Such conditions will be referred for specialty pain management. We do not stock or maintain a supply of controlled substances.
